
Practice Networking is a way for you to connect with other practices to share improvement work that you have done and to learn from others. Use the list of topics below to see what practices are willing to share and what topics they are interested in learning about. It's as easy as 1-2-3!
1) Find a practice that is willing to share (host a site visit) the information you are looking for. Just click on the topics below.
Is your practice listed? Download our Practice Networking Application and email/fax it to us and we will add you.
2) Contact the practice to set up a meeting.
Share/Host: These are practices that are willing to share what they have learned. If you are hosting a practice, please see our Host Checklist to help you prepare.
Learn/Visit: These are practices who want to learn more about a topic. If you are going to another practice to learn from them, please see our Visitor Checklist to help you prepare.
3) Take the survey. After you have hosted or visited a practice, please take a few minutes to complete our evaluation survey. We value your feedback and will use it to improve your practice networking experience.
