Practice Improvement Series Meetings (PRISM)

A Partnership of MaineHealth and the Maine Physician Hospital Organization

Practice Improvement Series Meetings (PRISMs) are held 2 times per year and are focused on improving chronic illness care (i.e. diabetes, CVD, depression, asthma/COPD), as well as offering cross-cutting topics appropriate for both physicians and office staff.

Open to all practice teams and healthcare professionals in the Learning Community, they are day-long sessions for practice teams to learn about the Care Model, quality improvement methodologies, and disease specific content. Local, regional and national experts are brought in to deliver education and skills training on the conditions targeted for improvement.

Welcome to the MaineHealth Learning Community's 2011 PRISM Series!
As we expand our focus into the Accountable Care concept, the series continued its alignment with the Institute for Healthcare Improvement's (IHI) Triple Aim concept of optimizing the healthcare experience within Population Health, Patient Experience, and Cost & Efficiency.
