Population Health Strategies for Primary Care
Presenter: Bonnie Zell, MD, MPH, Sr. Director- Population Health, National Quality Forum, Washington, DC.
Dr. Zell will describe population health concepts and how to integrate population level assessments and strategies into healthcare and primary care, specifically. Examples of primary care system redesign and collaboration between healthcare and community partners will be shared.
Goals: - Understand how to bring population level assessments, strategies and measures into healthcare

The panel discussed the effectiveness of national quality programs on population health work, and the impact of evidence based guidelines on how patients are cared for and how patients ask about their care.
